• Write down the names of your team members.
• Use a list of questions you’ve written down. Ask your most important questions first. Even if you cannot get all of your answers the first visit, having a list will help you keep track of the answers.
• Talk with your team about what health issue to work on first.
• Use your own words to repeat back the things you’ve discussed with your team. This way, both you and your team will know the information is clear.
• Ask your team about how to reach them after hours.
• Before you leave the office, be sure you know the things you need to work on before your next appointment.

The National Committee for Quality Assurance (NCQA) is a private, nonprofit organization dedicated to improving health care quality. The NCQA Patient-Centered Medical Home program reflects the input of the AmericanCollege of Physicians (ACP), American Academy of Family Physicians (AAFP),American Academy of Pediatrics (AAP) and American Osteopathic Association (AOA) and others. It was developed to assess whether clinician practices are functioning as medical homes and recognize them for these efforts. The NCQA Patient-Centered Medical Home standards emphasize the use of systematic, patient-centered, coordinated care that supports access, communication and patient involvement.
